PAYMENT/INSURANCE
We participate in most major insurance plans and bill your insurance carrier as a courtesy to you. Copays and deductibles are the responsibility of the patient. Because insurance plans vary greatly, it is your responsibility to contact your insurance carrier to determine eligibility, copay, deductible and referral requirements prior to your appointment.
Please bring your insurance card(s) and photo identification to all appointments.

Please note we are out of network with the following plans: BCBS Local, Cigna Connect Network, Duke Select, and Duke Basic .

PAYMENTS
Payments can be made by cash, MasterCard, VISA, American Express or Discover. Patient balances are due immediately upon receipt of statement. There will be an additional minimum $15.00 re-billing charge on any outstanding balance if payment is not received within thirty (30) days unless previous arrangements have been made in advance with our Billing Department.
OTHER TYPES OF PATIENTS
The Dermatology and Skin Surgery Center at Creedmoor welcomes self-pay patients. For patients with no insurance, payment is due at the time of service.
Cosmetic procedures will not be submitted to your insurance company. Payment is due at the time of service, prior to any treatment.
For all services rendered to minor patients, we will hold the parent or guardian accompanying the minor responsible for expenses incurred during the visit.
